Referrals for hospice are only made with your permission.

You or a family member can contact hospice to request service or you can ask your home care nurse, doctor or hospital staff to contact hospice for you.

When hospice is contacted the coordinator will set up a meeting to discuss the kinds of supports that would be most helpful for you and your family.
